Job Description:
Progressive Center for Independent Living (PCIL) is a leading non-profit organization dedicated to empowering individuals with disabilities to achieve independence, inclusion, and full participation in their communities. Our team is comprised of caring professionals who are dedicated to providing personalized services to enhance the lives of those we serve. We are looking for a dedicated passionate individual who is committed to providing exceptional support and guidance to individuals with developmental disabilities and their families. As a DDD Support Coordinator, you will be pivotal in assisting individuals with disabilities in Hunterdon County. You will be responsible for advocating for clients' needs, facilitating access to DDD services and programs, and collaborating with other service providers to ensure comprehensive support. Your primary goal will be to enhance the quality of life for our clients by promoting independence and self-determination. You will conduct comprehensive assessments of the client's needs and goals, and develop individualized service plans in collaboration with clients and their support network, coordinate access to DDD services, including community-based services, healthcare, education, and vocational programs, advocate for clients' rights and entitlements within the DDD system, monitor the delivery of services, conduct regular assessments, and make necessary adjustments to ensure clients' evolving needs are met, and maintain accurate and up-to-date documentation of all interactions and services provided. The salary range for the DDD Support Coordinator position in Hunterdon is approximately $45,000 to $60,000 per year, depending on qualifications and experience. Experience Required:
Bachelor's degree in Social Work, Psychology, Counseling, or a related field. Minimum of 2 years of experience working with individuals with disabilities and familiarity with DDD services and regulations. Strong communication and interpersonal skills to effectively engage with clients, families, and service providers. Exceptional organizational skills and attention to detail for accurate record-keeping. Ability to work independently and as part of a collaborative team.
